Good Faith Estimate of Services
Effective Date: September 18, 2025
Under the No Surprises Act, you have the right to receive a Good Faith Estimate (GFE) explaining the expected costs of non-emergency medical and mental health services.

You have the right to request and receive a written Good Faith Estimate before scheduling any service or upon request.

The GFE will include an estimate of the total expected cost of services, including counseling sessions, wellness services, and any related fees.

This estimate is not a contract and does not obligate you to receive services from Wholistic Care Counseling and Wellness. Actual costs may vary based on your treatment plan and therapeutic needs.

If you receive a bill that is $400 or more than your Good Faith Estimate, you may dispute the bill.
